Chapter 7: Somatic Oncology & Cancer Genomics

Cancer is fundamentally a disease of the genome driven by acquired somatic mutations. Somatic variant calling differs dramatically from germline discovery: tumor biopsies are heterogeneous mixtures of malignant clones, normal stroma, and infiltrating immune cells. Consequently, somatic variants often present at low Variant Allele Fractions (VAF < 5–10%), requiring specialized statistical models to distinguish true mutations from sequencing noise.

The Role of the Panel of Normals (PoN)

A Panel of Normals (PoN) is constructed from sequencing dozens of healthy, non-cancer individuals using the identical library prep chemistry and sequencing instrument. Mutect2 uses the PoN to eliminate recurring technical artifacts, PCR errors, and common germline polymorphisms that might otherwise masquerade as somatic mutations.

Common Pitfalls & Troubleshooting Checklist

  • Java Heap Exhaustion (GATK / Picard): If processes terminate with java.lang.OutOfMemoryError, set your environment variable explicitly: export _JAVA_OPTIONS="-Xmx16g -Djava.io.tmpdir=/tmp".
  • Index Timestamp Mismatch: If tools complain that a BAM index is missing or out of date, regenerate with samtools index -@ 4 sample.bam.
  • Conda Channel Conflicts: Avoid ABI incompatibilities between bioconda and conda-forge by setting strict priority: conda config --set channel_priority strict.
  • Chromosome Naming Contamination: Always verify whether reference FASTA headers use chr1 or 1; divergent naming causes silent alignment drops.
